Brief Answer:
See your doctor or neurologist.

Detailed Answer:
Hi Dear,

Welcome to HCM.

I have read your query and understand your concern.

There can be many reasons for involuntary lower lip movement or pulling sensation in lower lip.

But in your son's case it seems that the sensation is occurring either due to muscle spasm or anxiety or initial symptoms of bell's palsy.

However,I will want to rule out some disease or conditions by doing

1) blood counts ESR,
2) serum electrolytes (for hyponatremia,hypocalcemia),
3) electromyogram EMG,
4) nerve conduction tests for Bell's palsy (paralysis of muscle of one side of face due to damage to facial nerve),especially when if these symptoms are getting worse as stated by you.

If it is due to bell's palsy then usually it will go away by its own within 1-2 months or in case if it is persistent then medicines must be taken.

I would suggest you to consult your family physician or neurologist for complete medical history and thoroughly clinical proper examination with proper treatment.

Hope your concern has been resolved.

Regards,
