What Causes Indentations On The Hairline Area?

Question: What could be causing small indents that feel bruised on my hairline and scalp.. I've been trying to massage this and the dents hurt a lot.. I've also seen Hairloss in some of the dented areas.. I've been using finasteride and dutasteride for about three years so it u likely to be mpb. I only use these meds as precaution.

Thanks for the reply.. The indents do not show up in pictures.. I can only feel them. However, I am wondering something else. About 1 year and 10 months ago, I developed an excruciating burning sensation on my left temple region/hairline corner.. Two days after this pain, I lost hair rapidly through that area and saw many derms and doctors. I was treated for seb derm, but my doctor has finally said that she doesn't know why I'm not responding and what I have sounds nerve related.. The skin where this took place is highly sensitive even to a breeze. When it happened, my hairs in that area felt like they were being individually ripped out and pricked/stabbed with a hot needle.

Now, do you think that I could have had hair loss and pain there due to shingles in the form of, "zoster sine herpete"? I really didn't have any blisters there at all, but eventually had little inflammed bumps there which only last a little while..

I can certainly understand your concern.

The possibility of shingles is rare as visible vesicle lesions are a must for a diagnosis.

The possibility of post herpetic neuralgia/PHN is high.

Additionally, this hair loss is a form of alopecia areata and not MPB(male pattern baldness).

I advise you to discontinue finasteride/dutasteride as these drugs are associated with side effects on fertility potential of the male on the long term usage.

Instead, I prescribe oral pills of PREGABALIN 75 for 2 weeks against scalp tenderness and burning due to possible PHN.

For the hair loss, I prescribe oral pills of XTRAGLOW(BIOTIN 10 mg) and Topical solution of Minoxidil for 3-6 months.This medications have a great positive impact on hair growth.

Please check with your physician if he shares my view and if can prescribe these pills to you.

It can't be alopecia are at because I don't have complete bald patches. My derm gave me a kenalog injection to treat the inflammation on one side and it did nothing and I'm actually losing hair where I was injected.. I really can't take this inflammation anylonger. NOTHING HAS HELPED and the only med that would stop it is something like cyclosporine. Topical immune suppressor drugs cause hair loss, as well as lyrica which you suggested orally.

I'm aware of dutasteride/finasteride causing fertility issue, but I don't get side effects from these medications and honestly, my hair and inflammation issue is more important.

Other than zoster sine herpete type shingles, the only thing else I can think that would be causing all of this is frontal fibrosing alopecia or lichen planopilaris.

I can certainly understand your concern.

I did not prescribe any Immuno modulatory agents to you.

MINOXIDIL is a cutaneous(scalp) vasodilator.This improves the blood flow to scalp and improves the hair growth.

PREGABALIN on the other hand is pain modulating agent , that can be used to reduce the pain and burning of scalp associated with shingles if any.This is a pretty safe drug.This drug is not an immune suppressor as you perceived.

The possibility of frontal fibrosing alopecia or lichen planopilaris is also high in your case.In such a case systemic/topical corticosteroids agents can have excellent activity against the scalp inflammation.

FINASTERIDE/DUTASTERIDE only prevent hair loss and does not reduce any inflammation.

Please check with your TRICHOLOGIST if he can prescribe antiinflammatory steroid pills to you for 3-6 months.
