What Causes Increase In BP Readings Despite Being On Toprol And Lisinopril?

Question: Hello,
I am on 50mg of Toporal and 20 mg of Lisinopril. My BP has spiked over the past few days to hi 90's over 160-190. 61 years old but an avid cyclist and just in the last 2 months every time i exercise, about 5 min into my workout I have extreme tightness in my upper chest. It goes away if I work through the exercise or rest for a few min. Seems to be getting worse with every exercise.
Thoughts?

Hello! Thank you for asking on HCM! Regarding your concern I recommend you to consult your cardiologist, as your complains seems compatible with ischemic heart problems. You should discuss with your doctor; a physical examination, a resting ECG, some blood tests (including cardiac enzymes), cardiac ultrasound, and eventually a stress test would be very helpful to exclude cardiac angina (and warm-up angina).
I recommend you to stop having physical exertion and cycling until you consult your local cardiologist. You must visit the doctor immediately, as that way you may prevent a possible heart attack to happen. You may try sublingual nitroglycerin in case of chest pain until you meet your doctor. In case of persistent and severe pain run to the nearest emergency medical service.
