Question: My brother has possibly had a stroke in rural Honduras. He is weak, having difficulty with balance while walking, sleepless, no appetite, and slight slurring of speech. He is 71 years old and family hx reveals deceased father age 76 from stroke preceded by several years of incidence of TIA. My brother is alcoholic. The doctor in Honduras has advised him to not drink and he was given meneparol and vertizin tablets bid and one aspirin His sxs more closely correlate with a stroke than issues with nephrology. He was not given medication for detox. I am concerned that detox may trigger another stroke. He has an appointment in Honduras with a doctor for kidney and liver function tomorrow. They are also doing a sonogram. I have found someone to accompany him to the US. He will arrive on Wednesday. What do you advise? Should he drink moderately or abstain? Is he in danger of having another stroke because he is in withdrawal? What medical attention should be provided for him upon his arrival to the US. We live in XXXXXXX MD near Johns XXXXXXX and wish to provide him the best medical care possible. Thank you.

I am sorry for the symptoms your brother is suffering
Those symptoms are related with stroke or metabolic disorder.
It is really hard to find the right diagnose without examination.
First you have to do some blood test to exlude aenemia and test liver and renal function.
You should absolutely schedule an brain CT scan and much better brain MRI.
In my opinion as I treat my patient he should avoid alcohol even in small dosage, diazepam can be used to avoid agitation.
Aspirin was a good decision to be started.
