What Causes High Blood Pressure Despite Taking Medication?

Question: 46 yr old male, history, 4 heart attacks, 3 stents placed, CKD stage 3, hyperkalemia, celiac disease, fasting glucose when he's having chest pains average 150. Hypertension, hyper-orthostatic vitals, just did a renal artery scan...tech had a very difficult time finding these arteries. His bp is high and uncontrolled despite 75mg 2x daily of metatropal. Cardiologist today saw him and he had bad chest pains when we arrived. EKG showed he was tachy, also stated anterior infarct very possible. Cardiologist, stated he was having muscle pain and didn't need a cardiologist. Bp, on meds fluctuates from as high as 195/140 then drops to 84/60! She said this wasn't important and said to come back in 4 months!
