Thanks for your query,based on the facts that you have posted it appears that you have frequency for urination ,pain while passing urine ,hematuria with raised PSA.
All these symptoms are mostly due to one of the following reasons
1) Enlarged prostate with secondary infection (UTI).
2) Cancer of the Prostate and need to be investigated .
3) Infection of prostate (Prostatitis.)
Please consult qualified Urologist for clinical and digital rectal examination and get following basic tests done to confirm the diagnosis.
1) Urine routine and culture.
2) Ultrasound scanning of abdomen and pelvis
3) Ultrasound Guided multicore Biopsy of Prostate.
In the mean while continue taking Tomsulosin and antibiotics like Ciprofloxacin twice daily .
Further treatment will depend upon result of these tests and final diagnosis.
