Brief Answer:
Looks to be Epididymo-orchitis.

Detailed Answer:
Hi
Thanks for your query.
To recapitulate: Male/23 - feeling both testicles hard and heavy - right epididymis looks bigger - pain on pooping - feeling gassy - not visited a Doctor - no medications tried...
Such a condition can occur in case of bilateral epididymo-orchitis, wherein there is an infection in the testes as well as epididymes and may be spermatic cord.
The infection is usually retrograde coming from urinary tract.
Hence I would advise you the following:
First of all send the sample of urine for routine, microscopy, culture and sensitivity before starting of the antibiotic to get the best possible result.
Blood for CBC, kidney function tests and other tests.
Stool tests.
Ultrasonography of the whole abdomen , particularly for genitourinary system.
Ultrasonography and color doppler examination for the scrotum.
Per-rectal digital examination for the prostate and examination of the scrotum.
All these thing will lead to a proper diagnosis and give proper plan of management.
In the meanwhile take plenty of oral fluids.
Apply scrotal support.
Consult a General Surgeon or an Urologist for proper clinical evaluation, examination, correlation between all the factors to get a proper diagnosis and plan a proper management till cure.
