What Causes Gurgling Noise In Throat And Wheezing?

Question: Hello. I've had a 2 to 3 times a day cough for about 4 years - about the same time I started taking Lisonopril for my slightly elevated blood pressure. About 4 months ago I started having a gurgling sound in my throat ONLY when lying down and ONLY at the end of exhaling... I hear nothing while inhaling... I can make it better by coughing up some mostly clear plhem - never have seen any blood. When I stand up - the wheezing/gurgling immediately goes away. I don't feel sick or have any other symptoms other than the usual stopped up nose. I used Vancanase and now Nasonex occasionally. I went to Care Now about 3 weeks ago got a steroid shot and took a Z Pak and it immediately stopped the lying down gurgling and wheezing for about 2 weeks but it has slowly started back. I'm on my 4th month of this going on. Any ideas? Thanks a million...

1. Lisinopril belongs to a class of anti-hypertensives known as ACE inhibitors. These are notorious for causing cough. The exact mechanisms are not very clear. Changing or stopping this medication is known to ease cough in many patients.

2. The wheezing/ gurgling sounds usually signify post nasal drip or a silent reflux. Esophageal manometry and a USG (Ultrasound) of the Abdomen is required to rule out any hiatus hernia (which is common and sometimes due to the chronic cough). A Videolaryngoscopy (VL'Scopy) is a simple examination that must be done. A Sinus X-ray can be scheduled.

3. A Lung (Pulmonary) Function Test, Chest X-ray, blood tests for eosinophilia and a consultation with a Chest Physician is recommended.

4. Let your MD review and change the Lisinopril. The above investigations follow if there is no relief from the cough or throat symptoms. An anti-reflux medication (such as rabeprazole with levosulpiride) can be started.

5. Upload these test and examination results for discussing further treatment options.
