What Causes Frequent Urination And Swollen Face And Legs While Treating Pemphigus Vulgaris?

Question: My father was diagnosed with pemphigus vulagaris in the month of October, 2016. By the time it was diagnosed pemphigus covered his scalp, underarms, upper body parts. As he is diabetic doctor admitted him and started treatment with 80 mg dose of prednisolon oral. Insulin shots were given to control diabetes, and he was discharged after 6 days. By 21st November he got most of the improvement, except at few places, jaws and nose. Dr. tapered the steroid dose to 60 then 40 and not it is 20 and with that he has also prescribed him endoxan 20 mg oral. Now the major problem is side effects he is facing like, frequent urination in nights, swelling in face and legs. Could you please suggest/confirm if current ongoing treatment is fine please.

Hello. thanks for writing to healthcare magic.I can understand your concern.

As per my opinion treatment is correct.

Side effects like swelling of face and legs etc.are always expected when a pateint is on oral steroids.(prednisolone). This will decrease once dose of steroids is reduced.

However a slow tappering is advised in case of pemphigus.Sudden dose reduction of steroids can lead to relapse of diseae.

I will advise you to stay calm and be in regular follow up with your dermatologist.

Endoxan is an immunosuppressive drug which helps in early dose reduction of steroids.

Only one thing I will like to add is consideration of newer steroids like methylprednisolone or deflazacort which have less side effects.Please discuss this point to your dermatologist.
