Question: have been on ketoconazole for 6 days, 200mg BID, had a bad night, am 76, male, up 4 times to urinate and have been having pain across the middle of back, about where the kidneys are located. Is this related to the medication? Do I need to call the MD?

Brief Answer:
it is very unlikely related to medication

Detailed Answer:
Hello. Welcome to Healthcaremagic.com.
I have read your query.
I understand your concern.
Ketoconazole is a drug meant for treatment of fungal infection. It may lead to various side effects like nausea, vomiting, gastric upset, pale stools. It has its main side effects on liver. It doesn't affect kidneys.
The symptoms of pain amd frequent micturition that you mentioned may be due to Urinary tract infection or stones in urinary tract.
It is unlikely to be related to Ketoconazole.
I will advise you to get your Urine routine and microscopy done to look for the infection and Xray KUB to rule out the possibility of stone.
Visit your doctor so that he can examine you and order tests for you.
You can share my opinion with him. I
hope it helps.
Take care.
