Brief Answer:
Detailed investigation is required.

Detailed Answer:
Hi XXXX,
Thanks for writing in to us.

I have read through your query in detail.
Please find my observations below.

1. Frequent urination 5 times per night might be due to acute or chronic infection or due to any urinary bladder condition due to which the urinary bladder is not able to get emptied completely.

2. Some infections are not completely detectable by the usual urine tests. I hope you have got urine routine and culture and sensitivity test done. If possible please upload a copy of your reports and I will review the same.

3. The oily substance that sinks to the bottom of the toilet might be a discharge that is generated from the urinary bladder or urinary passage.

4. If you were my patient then I would have requested you to get an ultrasound scan of abdomen and pelvis done to assess the kidneys and urinary bladder. If you have got it done then please upload the report and if not then please discuss with your doctor and get it done if recommended after clinical examination.

5. Ciprofloxacin is an antibiotic and is prescribed for infections. Please take complete course and let me know if there is relief.

6. You have not mentioned any detail about recent sexual activity. Sometimes such symptoms might occur from sexually transmitted diseases and specific tests are ordered to make confirm the diagnosis.

7. Please drink adequate amount of water in the day and visit washrooms frequently to flush your urinary system.

Hope this answers your question. Please feel free to correct any oversight in my interpretation of your problems and discuss them in detail as per your requirements.

Hope your query is answered.
Do write back if you have any doubts.

Dr.Vivek

Brief Answer:
Please continue taking complete course of ciprofloxacin

Detailed Answer:
Hi XXXX,
Thanks for writing back with and update.

1. It is possible that you gave urine sample for culture after starting ciprofloxacin. In this situation, the culture report is negative even if infection is there. The result is negative because the ciprofloxacin killed many of the bacteria in the urine sample given for testing.

2. However if you gave urine sample before starting ciprofloxacin then the infection might be due to uncommon bacteria and not detected on culture.

3. Please take the complete course of ciprofloxacin and watch if your symptoms improve.

4. If there is no improvement after taking ciprofloxacin then I suggest that you get an ultrasound scan of abdomen and pelvis done.

5. It will also help to discuss in detail with your doctor about your sexual behavior and get tested for the possible sexually transmitted infections.

6. The discharge that sinks to the bottom of the toilet might be some type of thick discharge. It is important to know if the discharge is present every time or only after you pass urine following sex and masturbation.

7. Please do not panic and get a detailed work up done for your symptoms.

Hope your query is answered.
Do write back if you have any doubts.

Dr.Vivek

Thank you again!
1 The oily thing is come with urine, if I drink a lot of water it is not visible to see it but if I drink a little water there is a lot oily stuff that sinks in to the toilet. After I started cipro I have to drink plenty of water but I can still see the oil things.
2 I am worrying that like you said if the cause is the kidney problem cipro may make it worse ( I guess for the kidney)?
3 5 years a go I had UTI and had treated with cipro after that my urination frequency increased more than normal 1 to 2 times per night.
4 Recently I have started drinking coffee a lot and one time I had a lot coffee and I urinate 22 times in one night, didn't sleep at all.
5 starting from the last 5 months t now I never had a good sleep at night because of pee pee problem. Could this be infection this long time?

Brief Answer:
Please find details below.

Detailed Answer:
Hi XXXX,
Thanks for writing back with and update.

1. There might be a possibility that the oily sediment is protein or due to occult urinary tract infection. Drinking little water might cause protein in the urine. This requires specific testing to be done. I do not intend to make you worried however a step wise approach is recommended.

2. Yes, the proteinuria might be related to infections or kidney problems. Drinking too little or too much water might cause proteinuria.

3. Urinary infections have a tendency to cause increase in frequency of urination.

4. The caffeine in coffee might increase night time frequency of urination.

5. Some infections might stay undetected for a long time. I suggest you get routine test done for urinary proteins and also get basic kidney function test like blood urea and serum creatinine. Doing an ultrasound scan will always be advantageous.

Hope your query is answered.
Do write back if you have any doubts.

Regards,
Dr.Vivek
