Question: Dr. Rishu Saxena, My primary doctor ask me to wait for Valsartan 320/12.5mg HCTZ and 5 mg amlodipine take effect before add nitrate.
I take my record my BP twice a day,my BP always low in the morning and high on afternoon.
I send you 2 attachment the chart and the table my last 20 days.How I know my true BP reading when the BP fluctuate as shown.Can you review it and give me your opinion about my health.Thanks.

Hello Dear,
Have gone through both charts.

Your peak systolic was about 170mm of hg.Target systolic blood pressure above age of 45 is 140mm of hg.As age increases arteies get thickned and stiff hence blood pressure is raised.
Fluctuation of blood pressure by 20-25m of hg is normal during day.
Usually blood pressure is high in noon and evening and decrease during night time.This is mainly because of fluctuating level of cortisol hormone during 24 hours better termed as diural variation.In some people this variation is more apparentas in your case).

Frankly speaking i don't feel you need to worry at present.Though B.P is sometimes on a bit higher side yet for your age its ok.Continue the medicines as adviced by your primary for 1 more month.Most likely things will get better in a month.

Apart from medicines do strenous exercise for atleast 45 minutes a day.

As of now no need to add nitrate,Just wait and watch for next 1 month.
