Brief Answer:
behaviour therapy, some drugs.

Detailed Answer:
Hello
Welcome to HCM.

I can u derstand your concerns about your wife.

In my opinion, the lesbiansim does have some biological basis to it. There are neurohormones responsible for such behaviour. Though its not considered as a disorder.

Or, it can be a part of some illness like bipolar disorder or OC spectrum disorder.

I completely agree with you that this issue needs to be addressed by psychological therapy or counselling.

In this case, behaviour therapy must be instituted. There must be some conflicts in her unconscious mind which should be identified. It may be feeling of inadequacy, lack of love, increased sexual desire, discomfort with one's gender etc.
These can be worked upon then.

She must also not be treated harshly by family members. This will provoke her behaviour.

Regarding medications, I would suggest to divalproex and an SSRI like fluoxetine (prescription drugs).
Divalproex will also prevent post-surgery seizure and act as a mood stabiliser.

Kindly consult a psychologist in your area and discuss this.

Hope this helps.

I wish her well.
Thanks.
