I would explain as follows:

Detailed Answer:
Hello,

Welcome back on HCM!

Regarding your concern, I would like to explain that your palpitations feeling may be related to a gastrointestinal disorder like gastro-esophageal acid reflux (GERD) or hiatal hernia.

Nevertheless, as you are taking antihypertensive drugs like Napril (Enalapril) and Natrlix (Indapamide), there exists the possibility of an exaggerated medication effect leading to transient hypotention or even hypovolemia.

In such case, I would recommend to closely monitor your blood pressure values (several times daily for a couple of days up to a week) and also your water balance (daily drinking and urine output).

If abnormal values exist you should consult with your prescribing doctor to modulate your actual therapy.

In conclusion, I recommend you taking gastric anti-secretor drugs like Pantoprazole 40 mg/day or Omeprazole 40 mg/day, etc. to relieve your acidity feeling and also to perform an ambulatory 24 hours ECG monitoring (Holter) for excluding possible arrhythmic events during those palpitations feelings.

You should discuss with your doctor on the above mentioned issues.

Hope to have been helpful to you!

Feel free to ask me again, in case of any further uncertainties.

Kind regards,

Dr. Iliri

Brief Answer:
My opinion as follows:

Detailed Answer:
Hello again,

Let me explain you that blood pressure monitoring on a monthly basis in a doctor’s office is not sufficient to judge about BP trends.

It is necessary a close BP monitoring on a daily basis (furthermore several times during the day) for at least a week.

Only that way, we can be sure which the real trend of BP values is during the day-night interval.

After concluding the above BP values results, it would be possible to correctly judge about the daily dosage of antihypertensive therapy.

Regarding GERD (acid reflux), as I mentioned above, proton pump inhibitors (PPIs) would be helpful in reducing your symptomatology and also relieving palpitations.

Drugs like Pantoprazole, Lansoprazole, Omeprazole, etc. may be used.

A fibro-gastroscopy is necessary to investigate for additional lesions.

You need to discuss with your attending doctor on the above mentioned recommendations.

Greetings!

Dr. Iliri
