What Causes Exhaustion While On Coreg?

Question: we are now getting a referral to a new cardiologist, my dad remains in bigemmny
excuse my spelling. he is on Benicar 40, Lasix 40, hct 25, coreg 6.25 twice a day, for his crohns he is on budesonide, and pentassa. , XXXXXXX and spirivia for his copd. all this started after a remicade infusion, my question is this, he is always exhausted, I have read this could be a side effect of the coreg, or could it be from his heart. he has been hospitalized twice, the hospialist said he was in heart failure, his cardiologist says no its the pvcs.

Procedure detail
Objective: Diagnostic evaluation for coronary artery disease.
Technetium 99m-Tetrofosmin was injected at rest and at peak stress.
Initial setup. A baseline ECG was recorded. Intravenous access -
verified or obtained. Surface ECG leads and blood pressure
measurements were monitored. Regadenoson (Lexiscan) stress test.
Stress testing was performed with regadenoson (Lexiscan) by
intravenous bolus, for a total dose of 0.4mgover 10.00sec, followed
by a 5ml saline flush. Study completion: The patient tolerated
the procedure well and was discharged from the lab.

Summary
Impressions:
Narrative
- 1. Abnormal. Findings consistent with ischemic cardiomyopathy
2. Medium, relatively fixed, moderately severe inferior wall and
inferolateral wall photopenia consistent with MI and small, partially
reversible apical wall photopenia consistent with periinfarction
ischemia.
3. The post-tress LV ejection fraction was severely decreased at 26%,
while the rest LVEF was moderate to severely decreased at 38%. severe
global hypokinesia with regional variability in contraction. No transient
ischemic dilation.
4. The patient did not experience chest discomfort. No diagnostic
stress-induced ST-T changes. Frequent PVC&'s with couplets.
5. PROGNOSIS: As per ACC NCDR(R) Registry data, the study indicates a
HIGH Risk, equating with a greater than 3% annual mortality rate.
Recommendations:
1. Conservative management and optimization of medical therapy
aimed at heart failure is recommended in this 86 year old patient.
2. Follow up with Dr. XXXXXXX XXXXXXX the patient&'s primary
cardiologist.

I have gone through your reports in details.
your father has been going through Left Heart Failure with associated Rhythm disturbances(PVC"s).
As of now, he is on correct medication.
Exhaustion in his case can be due to LVF and Dyselectrolemia.
So avoid stress and anxiety.
Give plenty of fluids.Repeat serum electrolytes again.
