Brief Answer:
Get done chest x ray and sputum culture for AFB.

Detailed Answer:
Hello XXXXXXX thanks for your question on HCM.

I want to ask few questions first.
1. Are you on Anti Tubercular Drugs? If yes than on which drugs
2. Have you undergone chest x ray in recent times (less than 1 month)?
3. Have you undergone sputum culture for AFB in past?
4. May I know your weight?

Blood in cough (haemoptysis) in your case can be due to
1. Friction bleeding
2. Reactivation TB
3. Drug resistant TB
So kindly reply me with above answers, so that I can guide you better.
I will be happy to help you further.
Wish you good health.
Thanks.

Brief Answer:
Get done chest x ray and sputum culture for AFB.

Detailed Answer:
Hello XXXXXXX thanks for the information.

In my opinion you should first get done chest x ray. And compare it with previous x rays. Look for any worsening.
If it is worsening than you may have drug resistant TB. So get done sputum culture for AFB and sensitivity too.

If x ray is improving and fibrosis is seen than no need to worry much. The blood may be due to friction from coughing. Still I advise you to get done sputum smear examination for AFB. And if this is negative, then no need to worry much.

If chest x ray is not much conclusive than better to get done CT THORAX as CT gives much better information than chest x ray.

Bronchoscopy and XXXXXXX (Broncho Alveolar Lavage) analysis for TB is also more specific than sputum examination.
So better to consult pulmonologist and discuss all these.

Hope I have solved your query.
