Brief Answer:
likely musculoskeletal unless heart disease risks

Detailed Answer:
Hello and thank you for your question.
It sounds like you may have a muscle or tendon irritation around the scapula that can cause pain and irritation. If this is the case then the best therapy for this would be stretching exercises conbined with physical therapy. Sometimes there is scar tissue or irritated tendons that can cause this kind of pain and itching.
However, if you have risks for heart disease (high blood pressure, diabetes, family history of early heart disease, high cholesterol, smoking) then you need to see a doctor and make sure this is not coming from your heart.
In cardiac chest pain it is usually triggered by activity and may be accompanied by chest pressure (though it can just be in the back), shortness of breath, sweating and anxiety.
If you are uncertain then see a doctor to make sure it is not your heart. Then they can also make a referral to physical therapy if indicated.
Regards,
Dr. Robinson
