Question: Husband diagnosed with ALS 10 + YEARS AGO. RECENTLY EATING VERY LITTLE, DRNKING ABOUT 32 OZ PER DAY. CAN'T PASS URINE UNLESS I PUSH ON HIS LOWER ABDOMEN. HE SAYS NO PAin. Urine dark, color of iced tea and smell bad. Output per day around 8-12 OZ. could this be related to ALS, or possibly something else??

It is due to ALS affecting the bladder muscles with secondary UTI.

Detailed Answer:

Hello,

Based on the facts that your husband who has been diagnosed with Amyotrophic Lateral Sclerosis is unable to pass urine voluntarily and he is passing coffee colored bad smelling urine.

This is due to the involvement of motor neuron controlling the bladder muscles (Neurogenic bladder) associated with secondary UTI.

Get done his routine urine test and urine culture to find out the organisms causing this infection and antibiotics to which they are sensitive to.

Give him broad-spectrum antibiotics like Cefixime along with urinary antiseptics like Nitrofurantoin and urine alkaliser thrice daily. Later on switch on to appropriate antibiotics as per culture report.

Get the prescription of medicines from your family Physician.

Ensure to drink more water to keep urine dilute and free from infection.

Putting a Suprapubic Self Indwelling Catheter will help to keep the bladder empty and thereby prevent infection. You can discuss this with your Urologist.
