Question: hi im 19 year old girl. from last 5-6 years my underarms have darken like black colour and are very itchy and smelly. what can be the remedy for it?

Hi

I understand your concern regarding your skin problem and I hope I am able to help you in the same.
I have seen the attached pictures .
You seem to have developed an infective eczema over your axilla leading to itching and pigmentation .
You should keep the affected area clean and dry . Wash the affected areas with a mild antiseptic solution twice daily .Mix and apply a topical antibiotic cream like mupirocin along with an antifungal cream like clotrimazole over the affected areas twice daily . Avoid using any deodorants over the area .
Take some oral antihistamines like Benadryl or levocetrizine along to relieve the associated itching and allergy .
Avoid touching or scratching the area since it might increase the infective eczema even further .

can you please tell me the exact names of both creams with its type like oz or something else etc and are they available in normal medical stores?

Hi again

The foul smell that you are experiencing is the result of infection in the area .
It is important to treat the infection first and then treat the pigmentation .
You can mix and apply candid b cream and t bact cream over the affected areas twice daily . Both of these are easily available.
Once the infection heals up completely ,which might take upto two weeks ,then you can start application of a kojic acid based skin lightening agent over the area twice daily to reduce the pigmentation.

hi again firstly thank you soo much my infection has totally ended but hyper pigmentation is still there can you please tell me any good kojic acid cream or something that reduces the pigmentation faster as i have some event in july.
secondly iam having back acne also and it is on shoulder and chest and they are leaving scars and blackness can you please tell me its cure specially for blackness and scars.

Take oral vitamin c tablet once daily .
Avoid rubbing the affected areas.

All of the above measures will slowly take care of the pigmentation .

Hi again

It is very important that you do not scratch the lesions of acne since picking of the acne lesions leads to increased chances of scarring .
You might need to apply some topical anti acne agents like clindamycin gel available as clindac gel or nioclean or clin ad gel over the active acne lesions once daily .
Use an anti acne or antiseptic soap for bathing .
Do not apply any other cream or oil over the affected area since these may increase the oiliness of skin and increase the acne lesions .
In patients with severe acne ,oral anti acne drugs like azithromycin or doxycycline have to be given along .

ques 1- which oral medicine should i take for back ance?
ques 2- can i use any back acne spray to treat my acne such as Glytone Back Acne Spray (2% Salicylic Acid) 8-Ounce Package or NEUTROGENA Body Clear Body Spray available in market or any other spray u suggest?
ques 3- i learnt that kojivit is very slow and people see minimal results so can i use any chemical peel for my pigmentation issue ?as i have an urgency is yes then which peel i should use?

Detailed Answer:
Hi

For acne ,you can take tablet azithromycin 500 mg once daily for three days and then repeat the course after two weeks .
You can use the salicylic acid based spray. It will help both in reducing the acne inflammation as well as exfoliate skin and reduce the scars.
Kojic acid is a mild lightening agent but since it contains natural products ,it is safe to use and can be used for as long as one wants . A stronger lightening agent would be a triple combination cream like skinlite or triluma cream which has to applied at night only over the affected areas .But it has to be applied minimally over the affected areas and should not be rubbed too much.
As far as chemical peeling is concerned ,salicylic acid based peels are best suited for acne prone skin with blemishes.
