Brief Answer:
Please do the following tests

Detailed Answer:
Hello and thank you for asking in HCM

I can understand your concern

According to the history you provide if he were my patient i would recommend him to do the following tests to exclude heart and lungs problems that may appear with this symptom:

ECG
TROPONIN CK-MB
Cardiac ultrasound
Chest X Ray
D-Dimer
Carotids ultrasound

It is very important to evaluate his actual heart and lung function.

There are several heart andlung diseases that may cause lightheadedness.

If the results come negative than we can say its stress if they come positive than the relevant specialists (cardiologist or pulmonologist would decide for he proper treatment and follow up depending of the problem)

Please discuss with his doctor for the above

Or if you want you can attach me the results and i will try to help you again.

Take care
