What Causes Congenital Diaphragmatic Hernia?

Question: 7 Years old male child undergone Endoscopy. on admission he was having Severe Iron Deficiency anaemia. The patient is suffering from XXXXXXX since 4-5 months.As per treating doctor pt underwent upper GI endoscopy which showed Stricture in lower oesophagus which is likely a peptic stricture developed due to continuous reflux as a result of Hiatus Hernia. Kindly confirm the exact cause of hiatus hernia in this case.
