What Causes Chest Pain, Severe Vomiting And Diarrhea?

Question: Hi,i am male,25 years old..i have pain in the middle of chest,and very uncomfortable stomach,severe vomiting including diarhea past 6weeks,i've been on omeprozole for 4 weeks 20mg morning and 20 mg night,still no progress and add up to 2 weeks on ome. and i stop because my symptoms seems looking fine also the reason was it make my stomach upset and my anxiety up..but after several days nw i have this bloated feeling,tight and burning chest,gurgling stomach and sometime sourtaste in my throat and mouth,the pain occur most in the middle of chest along through my back,all this pain goes along with my lightheaded and sometime confusion and heart palpitation,hard to get sleep and awake early because of that,my stomach was very disturb and bowel very active at the morning..i never checked on h.pylori..is h.p is the major culprit of this,and can i have opinion on this?thank u very much..

Thanks for putting your query in HCM. I am a Gastroenterologist (DM).

What all you have mentioned like burning chest, sour taste in throat, pain in middle of chest and back and even palpitation suggest that you are having reflux disease. Treatment for h.pylori leads to eradication of h.pylori which actually aggravates the symptoms. There is lots of literature suggesting that eradication of h.pylori aggravates the symptoms of reflux. So there is no need of check and treatment for H.pylori as it will rather aggravate your symptoms. Also anxiety leads to increase in acid production so keep calm.

thank u for the explanation doctor.Again,about the reflux where did i get this from?,can i know how to get rid of this?bcause this reflux disease put all my works quality bcome low,i bcome more moody,tired most of the time..

Reflux is most commonly due to abnormalities of lower oesophageal sphincter

Detailed Answer:
Welcome back. There are many causes for the reflux disease. Most important of which is abnormalities of lower oesophageal sphincter. What I suggest you is, which I found very useful in my patients is:
1.Avoid fatty and spicy meals
2. Don’t lie down soon after meals for at least two hour
3.Take small and frequent meals, instead of full meals at a time
4. wear loose clothes
5. If you are overweight or obese do weight reduction with dieting and exercise
6. Avoid smoking and alcohol if you take
7.Don't drink water with meals
Among the medications:
1. take domperidone it don't simply act as a prokinetic agent but it also increase the tone of lower esophageal sphincter which most of the time is found to be relaxed on endoscopy in patinets with reflux disease
2.If you are relieved completely with omeprazole then may take another proton pump inhibitor like rabeprazole which is more potent
This all I found very useful in my patients with reflux disease. I hope I have answered your query. Wish you a good health
