Brief Answer:
Vit B1 and B12 deficiency

Detailed Answer:
Hi,
Thanks for asking.
I am Dr. Prakash HM and I will be answering your query.

Based on your query, my opinion is as follows:

1. Beriberi (specifically dry beriberi) or peripheral neuropathy.

2. Dry beriberi secondary to Vit B1 (thiamine) deficiency secondary to malnutrition due to diarrhea and also previous malnutrition (alcohol, selective food intake, etc.)

3. Peripheral neuropathy due to Vit B12 deficiency.

As there is no exposure to cold or warmth and changing of climate is not improving conditions, Raynauds phenomenon cannot be thought of.

Vit B -complex injections, should improve the symptoms, as the pain appears to be neural in origin.
As I am not sure about other chronic diseases, you might have or medication history, based on symptoms, if you were my patient, I would advise Vit B1 and B12 injections to improve vitamins immediately.

For pain, pregabalin or gabapentin may be helpful. You need to take prescription from your doctor.

Hope it helps.
Any further queries, happy to help again.

You are welcome

Detailed Answer:
Hi XXXXXXX
Thanks for replying.
You are welcome.

Do get these shots. Hopefully, I am correct.

Diet controlled diabetes is excellent. Keep testing with HbA1c, every 3-4 months, to see the amount of control.
