Brief Answer:
Possibility of anal fissure needs to be considered

Detailed Answer:
Dear Ms

From the description given following are the causes of your symptoms

1. Anal fissure : Possibility of anal fissure needs to be considered in view of pain and burning sensation associated with passing stools. Underlying constipation is predisposing factors. Passing of hard stools is associated with tear at anal opening leading on to anal fissure. Exact diagnosis will depend on clinical examination

2. Associated Urinary tract infection : Diagnosis is based on clinical history of burning sensation associated with passing of urine

Following are the predisposing factors

1. Decreased intake of water

2. Decreased intake of fiber in diet

3. Lack of physical activity and exercise

4. Stress and anxiety

I would suggest you the following

1. Ensure good intake of water : 2 to 3 liter a day

2. Urine alkanizing agent like blueberry will be helpful

3. High fiber diet : 2 servings of vegetable salad and 2 servings of fruits especially citrus fruit will ensure adequate intake of fiber

4. Hot Seitz bath : To a tub of lukewarm water add 2 spoons of antiseptic solution. Sit in it for 15 to 20 minutes each session twice a day. It will help relieve pain by relaxing perineal muscle

5. Laxatives like Lactulose will be helpful in treating constipation

6. Urinary analgesic like Phenazopyridine will help relive burning pain

Do get back to me if you need any further assistance, will be glad to assist you.

Take care

Best regards
Dr Deepak
