Brief Answer:
Start antiinflammatory drugs and visit his doctor....

Detailed Answer:
Hi,

It would be difficult to predict the cause of this pain without looking at the wound. My best guess is increase inflammation around the site of wound. But it doesn't explain the relative pain difference in different positions.

You can try over the counter pain killer such as aleve before visiting the ER.

In any case, he needs to visit his surgeon who can evaluate the current status of wound healing.

Let me know if you need clarifications.

Regards
