What Causes Brownish Semen Discharge And Abdominal Pain?

Question: I have had epididymitis before and at one point when I was around 19 or 20 I was put on antibiotics for an infection that had developed in my scrotum. However, I am 34 years old and at the beginning of this year I was injured when my dog jumped onto the couch and I was wearing lets just say relaxed underwear. A couple weeks after this injury I developed some swelling and at the time I had no PCP. I decided to ice/keep elevated and use Ibuprofen.
It went away and I eventually was feeling well again. However, during this period of time I had some brown in my semen when having intercourse with my fiancee. After the swelling went away and I started feeling better I did not notice the "brown" anymore. My doctor from when I was younger told me if I am doing anything where I need to having a lot of motion like "jogging/etc." that I should wear more restrictive underwear but I did not listen. Recently I have been doing aggressive workouts and I believe I re-aggravated the epididymitis. However, can this cause a slight pain in my abdomen? Also, can I have an infection because when I noticed an issue was recently I made had intercourse and checked again and it was brown. I am wondering if I have an infection in my bowels or a virus... I have a physical scheduled for the 28th. Do you think I may need antibiotics to help clear it up? Also, what is your recommendation in terms of me protecting my low hanging testicles?

Dear Mr XXXXXXX

Only the symptom of alteration in colour of semen does not suggest the possibility of Epididymitis or any bowel infection. Bowel infection will not result in discolouration of scene.
Epididymitis is suspected based on clinical history of pain and discomfort in the testis sometimes associated with burning sensation while passing urine. There could also be referred pain in the umbilical region.

Diagnosis of Epididymitis is confirmed by clinical examination and Ultrasound. Decision to start antibiotics is based on it. I would suggest you to consult your Physician for detailed clinical examination.

Testis may be high or low hanging depending on surrounding temperature. It is low hanging when the surrounding is hot and humid. The recommendation is wearing right size of underwear which securely supports the testis.

Oh. So Epididymitis cannot cause bleeding? I was told it could so that is why I thought that.
Oh. I did not explain either that I do have tenderness and my van defrens or the tube that connects to the back of the testicle is sore and inflamed.. Also, i have fluid in my scrotum. I will do as you have instructed and just continue with my visit to the physician.

I assume you meant you have fluid around the testis. It is likely to be hydrocele. As previously advised exact diagnosis will depend on physical examination.
As you have now mentioned that there is tenderness of the testis and in light of fact of fluid(Hydrocele) possibility of Epididymitis needs to be considered.

I would suggest you the following

1. Good scrotal support: Right size well fitting underwear
2. Rest
3. Avoid strenuous physical activity and advise
4. Analgesics/Anti-inflammatory drug like Ibuprofen


Adequate rest,analgesic, scrotal support will reduce the inflammation in majority of cases.
Antibiotics are only indicated under following circumstances

1. Documented Urinary tract infection
2. Infected secondary hydrocele
3. Severe infection as documented on Ultrasound /color doppler scan
