What Causes Boils On Buttocks After Intercourse?

Question: Hi, I had sex with a sex worker on 1 XXXXXXX 2015. Five days later I started developing some boils on my right buttocks which are extremely itchy and irritating. During sex, I wore a condom which slipped off during intercourse and which was later pointed by the sex worker that the condom had slipped. She was extremely disappointed that the condom had slipped (might be an indication that she was either worried that i have std/hiv or because of pregnancy chances). So, I dont know exactly for how much time I had unprotected sex with her after the condom slipped. I am extremely worried. Further, I we also engaged in giving each other oral sex (when she gave, the condom was intact). Pls let me know the way forward.

I have gone through your query. By considering the symptoms you mentioned and incubation period this can be most likely genital herpes. It usually come as blistering type of rash associated with burning sensation or itching. It is better to rule out that by showing your GP.

Other possibilities like fungal infection also should be ruled out. Viral culture can be done from the blisters for identification. In case of this antiviral therapy with drugs like famcyclovir will be helpful. Acyclovir ointment for local application also is done for this. Ice pack application or washing with ice water gives soothing from burning sensation.

As you had unprotected sex you should go for a STD tests. HIV, VDRL and HBSAg also should be done. HIV antigen test or PCR test can be done after 3 weeks to 1 month after exposure.

Hi Doctor,

Appreciate your response.

I have the following questions:

1) You have mentioned that it is 'beneficial to rule out that by showing your GP'. What does it mean? Does rule out mean that I have the above disease?

2) Is genital herpes life threatening like hiv or any other std?

3) My family doctor gave me some tablets and T-Bact cream to apply on my right buttocks region. The boils have subsided to some extent. Should I continue the same? If no, pls suggest me some medication and the period of consumption.

3) Can these boils appear again after they have subsided completely?

4) Can you please give a list of tests I should do (pls tell me when I should do these tests from the date of exposure)

5) If I have hiv, does that mean that I cannot get married, or have sex with another partner, or lead a normal healthy life?

I am extremely worried and I completely regret my act.

Kindly reply in the order of the questions asked in layman language (to assist me in understanding).

Thank you for your follow up query.

1. What i mentioned was i suspect genital herpes infection. But it can be just a fungal infection also. That is why i told you to show your General practioner and find out exactly what it is.

2. Genital herpes is not life threatening diesease. It is some thing like a cold sore which comes and go on lips. This will go away and may come back when your body immunity is low. The irritation, burning sensation and itching is the main problem associated with this. If you have this then you can spread to some one by sexual contact even though clinically you are not having it.

3.Genital herpes usually subside by its own. But anti viral drug therapy will decrease the duration and severity. I advise my patients with famcyclovir tablet for five days and acyclovir ointment for local application. T bact you are using is a antibacterial ointment . It can help in preventing secondary bacterial infection in such rashes.

4. Usually atleast one month is needed to get sensitivity for most std tests. HIV antigen test or PCR can be done after 3 weeks. But HIV rapid should be done after 3 and 6 months to confirm it better. Gonorrhoea , chlamydia and syphilis can be done after 1 month also. Urine sample and urethral swab helps in testing gonorrhea and chlamydia. VDRL test will help in testing for Syphilis. PCR blood test and cell culture test can be done for herpes. HbsAg test should be done for hepatitis B also. This also can only be done after 1 month.

5. You don't need to worry about HIV. Hope for the best. Get a counselling to relieve tension. In case of HIV spread it is better to have a partner with HIV. Otherwise only extreme protected sex is the only possibility with non infected ones.
