Question: My son's friend just called and asked for help. His scrotum and penis are black and blue. I think this happened suddenly and without trauma to the area. He said he talked to an EMT and he told him it was a hernia. He does not have health insurance but I am trying to determine if this is an acute emergency or if he can wait. I have read some information about the chance of strangulation of bowel in the scrotum. He is a musician and heading to XXXXXXX now in a van. They have a long trip today to get there.

It is very difficult to think of a specific diagnosis with the information you have provided but if it was an acute condition like strangulation of inguinal hernia or a blood circulation problem ,he would have very strong ,unbearable pains .
If your son ,despite this change in color of his genitals ,feels good (no pain ,no swelling,normal urination etc),it is not an emergency but it is very important to see a doctor as soon as possible.It can be due to a minor trauma or a varicocele.Physical examination is essential for the diagnosis.

It's not my son but my son's good friend. I wanted to add that this young man has a hard lump 3 inches to the left of his belly button that he can feel.

Without being able to examine the area, it is difficult to say exactly what this lump is but considering the location it can be a hernia of the abdominal wall,a cyst ,lipoma etc.
If he doesn't have acute symptoms like abdominal pain ,genital pain, abnormal urination or defecation ,it is not an emergency but he need to see a doctor as soon as possible (if the symptoms persist).
Physical examination ,some blood tests and abdominal ultrasound are necessary to understand something more ,regarding the cause of these symptoms.
