Detailed Answer:
Hello. Thank you for writing to us

I have read your question and I have also reviewed the Image.

Nodulocystic acne, specially along the jaw line, in a female and specially if recurrent Or relapsing as soon as treatment is discontinued could be due to underlying hormonal imbalance.
Do you have any menstural irregularity? Any coarse facial hair? Or hair loss and thinning from scalp?

Regards

Thank you for getting back to me. No i do not have any of the symptoms you described however i have switched my birth control from oral contraceptive (Lybrel) to the Mirena IUD. I am thinking this might be the culprit but i am unable to do anything about that.

Brief Answer:
Levonorgestrel in mirena is androgenic and can cause acne

Detailed Answer:
Hi.

Mirena IUD contains levonorgestrel which is an effective contraceptive but is an androgenic progesterone i.e it can cause acne as a side effect.
Lybrel contains ethinyl estradiol (an estrogen) in addition to levonorgestrel which counters some of the androgenic side effects of levonorgestrel and is therefore better than plain levonorgestrel.
However in my view switching to a non-androgentic progesterone like drospirenone in combination with ethinyl estradiol e.g Yaz Or XXXXXXX would benefit your acne in addition to birth control.
In addition you may use a topical otc antiacne preparation like benzoyl peroxide+clindamycin combination gel/ lotion for your acne.
