Brief Answer:
Need personal care, prevention of reinfection & expert advise.

Detailed Answer:
Hi XXXX,
Thanks for the query under Health care magic.I have read it, seen the images sent .. & have understood your concern.
Actually itching in groin region is mostly from Jock itch... which is a fungal infection.. received due to skin contact with infected person or using clothes of infected person/ moist skin due to sweating/ friction caused by tight fitting clothes / Lack of personal hygiene../ Obesity .. as the fungi thrive in skin folds.
* You had tried anti fungal medication ( ? over the counter ointment )for several moths without a relief.. that makes us think of causes of failure of recovery.
- The additional care for removing fungus was neglected - cleanliness( cleaning with soft soap & water/ using loose hosiery undergarments, avoid using tight clothing, cleaning of undergarments carefully & drying them completely.. preferably in sunlight/ reduction of weight / avoiding reinfection
- Severe infections do need oral anti fungal medicine along with local care with a doctor's consultation... which was not taken.
- Health condition like Diabetes causes failed recovery.. it needs to be excluded.
- Other possible causes of itching which do not respond to anti fungal cremes like- allergy, contact dermatitis, eczema need to be excluded or confirmed & treated specifically / excluded.
* You had used alcohol application which acted as a counter irritant .. & relieved the itching for time being The basic cause was still remaining .. thus there was scrotal lesion. The scrotal lesion seems to be secondarily infected ( yellowish plaques seen )causing inflammation & pain.
* In this context I would suggest consultation of a dermatologist for locating exact cause & specific treatment.
Till then you can follow personal care as described above & apply a combination of antibiotic skin ointment along with your anti fungal ointment.
* Thus an expert help along with personal care & prevention of reinfection you would be relieved of the problem.
I hope that answers your query . Any follow up query is welcome.
Thanks.
