Posted on Fri, 11 Mar 2016
Question: I have a blood clotting disorder (antiphospolipid antibody syndrome) and I feel and got a large bruise on my left thigh today. It is pretty dark and purplish/green. I am suppose to fly to Costa Rica (a 4 hr then a 2 hr) flights in 5 days. Is this safe and should I take any special precautions?

Brief Answer:
Consult your hematologist. Bruise needs evaluation first.

Detailed Answer:
Thank you for asking

I read your question and i understand your concern. With antiphospholipid syndrome you are at hypercoagulable state and it needs constant anticoagulation This bruise on your thigh you provided could be a sequela of that and needs work up like doppler sonogram and bleeding profile to sort it out.

regarding the travel to costa rica, a flight of 4 hours and then 2 hours is not that long , just wear a compression stockings and keep the limbs mobilised during the flight. But get a green signal from your doctor first as the bruise needs evaluation first .

I hope it helps. Take good care of yourself and dont forget to close the discussion please.

Thank you. I went in and had my PT/INR checked this morning so that should give me a good indication of what my blood levels are at. I take alternating days of 6/7 mg of Coumadin. I will be sure to wear my compression socks but should I wrap my thigh with an ACE bandage or anything so that the area of the bruise is compressed as well? Will exercise make the bruise/injury worse?

Needs management,

Thank you for getting back to me

Exercise won't make the bruise injury worse, it will rather help to prevent further clots. However deranged PT INR or other bleeding profile derangement needs to be corrected as that caused the bruise. COmpression stocks are enough and no need to wrap the thighs . If the PT INR comes above 3 , reduce the warfarin dose to half the pill. Stay in touch with your hematologist and let them decide what is best for you.

I hope it helps.
