However I would like to know if
1. Are you a diabetic?
2. What's your age?
3. How long have you been with the symptoms?

Here's why I ask

1. In case you are a diabetic, it could be a heart attack. In diabetic patient, the typical feature of chest pain doesn't always happen. There are variable features like yours. Radiating discomfort to arm is why I consider it. And is more common in elderly.

2. Could be a gastritis. This may be sudden or prolonged. Can affect young and old. May have similar symptoms. Sometimes GERD may be present.

3. Could be a cervical and dorsal radiculopathy. This could be both sudden or prolonged. But usually prolonged. And have typical features like what you described. Could also be brachial plexitis.

So please consider these. You need a urgent cardiac evaluation, can start pantoprazole 40 mg once daily before food. And if cardiac condition is ruled out, an MRI Of spine may be considered.
