Brief Answer:
Gum infection, scaling, repiratory, GIT disorders

Detailed Answer:
Thanks for your query, I have gone through your query.
As far as the bad breath or halitosis is concerned, I have the following possibilities.
The first possibility can be because of the gum infection secondary to the deposits over the teeth or because of food lodgement. Even though you brush your teeth and even after that you are getting bad breath means, it is because of the gum infection, so consult a oral physician and get your teeth cleaned once and maintain oral hygiene. You can use mouth washes.
The other possibilities can be because of the respiratory tract or gastro intestinal tract disorders, these can result in halitosis or bad breath. For this you need to consult a ENT and Gastroenterologist respectively.
I hope my answer will help you, take care.
