What Casues Severe Abdominal Cramps And Vaginal Spotting After A D And C Procedure?

default
Posted on Mon, 15 Sep 2014
Question: My wife had a d&c 1 week back ... Pregnancy duration was only 8 weeks... But the ultrasound showed only a 10 mm gestational sac and a faint yolk sac...
It was only after getting d& c done we read about asermans syndrome and were horrified with the % occurance.

She was having normal period like cramps with sometimes a more severe cramp, and lite spotting. ... However yesterday she also felt a bit bloated..which subsided today..but the discharge was watery pink... And not brown ..Does this mean it is fresh blood coming out of a scar caused during d&c? And does the bloating indicate infection? If so what care should be taken? The Gyne who did the op isn't saying anything ... Just gave her some probiotics. She took amoxyline for 5 days after d&c.

Hello sir, thanks for trusting health care magic.
I'm happy to see your inquisitive attitude towards this issue. Keep it up but don't be sad after reading internet and just discuss it on health care magic and you will get sincere advice over your issues.

Regarding incidence of asherman d&c it ranges from 16-25% depending on the expertise of doctor and gestation of pregnancy and the adhesions may vary from mild flimsy adhesions ( more common) with no effect on menses or pregnancy or less commonly causes excessive adhesions which are serious.
Neither the pinkish discharge nor bloated feeling is suggestive of infection.
Both of these symptoms are very common after abortion.
Moreover you had a planned d&c with an antibiotic cover after the procedure so practically nil chances of asherman or infection.
After fifteen days of d&c just visit your gynecologist for physical examination and vital check.
All the best.

The discharge is pinkish .. Which means it is fresh blood .. Could it come from an injury caused during d&c? If not what is causing it?

Brief Answer:
nothing serious

Detailed Answer:
No matter how meticulous a doctor is there is definitely some decidua which is left inside the uterus which gets out of vagina as discharge for about 3-7 days and at times even more so don't worry about the discharge.
Injury definitely occurs in all d&c procedures but is usually harmless and is a part and parcel of the procedure because it is a blind procedure
I hope I have answered all your questions but in case you have any other concern I will be happy to help.

But The remaining blood in the uterus should be brown because it is old blood , but this is pink indicating injury?

Brief Answer:
normal to see such bleeding

Detailed Answer:
When brown blood comes it is old collected blood and if it is a pinkish discharge it is due to the bleeding from implantation site of the baby which when gets expelled can bleed actively for some time till uterus retracts to normal size.
So just relax and wait for the blood to stop and ask her to take rest and drink lots of water daily.
Wish her good health.

Detailed Answer:
Hello sir I would like to know if
- she has foul smelling discharge per vaginum
- any bleeding and type of bleeding like dark altered blood or active red bleeding
- are her breasts tender or filled with milk all the time?
Take a repeat reading after 12 hours of previous one and in case she still has fever then she needs to be put on an antibiotic and an anti inflammatory drug.
Till then ask her to take ample amounts of water and apply a sterile vulval pad so as to quantify the bleeding if any.
Waiting for your response.
Take care.
