default
Posted on Wed, 20 Mar 2019
Question: pain and swelling in anal tract between anorectal line and sphincter, can't insert suppository or PreparationH applicator without anus pain.

Also, visit's to doctor only resulted in recommendation of laxatives, ignored my report of pain in the area.

(please resend answer in case it was accidentally counted as spam and deleted. Thx)

Thank you.

Are there US equivalents for those drugs?

Also, patient is on kidney dialysis for 8 years, would that affect your diagnosis and recommendations?

Thanks

Brief Answer:
You have to use a potent anti-inflammatory.

Detailed Answer:

Hello,

As per the history was given by you most likely you are suffering from proctalgia.

Proctalgia is a severe, pain in the regions of the rectum and anus especially on passing stools or inserting things in the canal. It can be caused by cramp of the levator ani muscle in the anus.

Treatment-
Take an anti-inflammatory like a combination of aceclofenac and serratiopeptidase along with potent anti-inflammatory like methylprednisolone in high doses for a few days.

Don't try to insert anything in rectum till you are completely fine.

Keep using laxative (stools should be soft and very easy to pass).

Follow up after 5 days.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Ramesh Kumar, Gastroenterologist

Brief Answer:
Follow up answer.

Detailed Answer:

Hello,

As the patient is on dialysis we can't give him any pain killer and anti-inflammatory drugs.

I would suggest him to take Tylenol and if not relieved consult the nearest doctor.

We can't prescribe medication to a person on dialysis for 8 years without even knowing parameters.

Hope I have answered your query.

Regards,
Dr. Ramesh Kumar, Gastroenterologist
