Heart disease is usually associated with symptoms such as increased heart rate, tight central chest pain, shortness of breath.
There is also usually a previous history of heart disease or conditions that would cause heart disease such as uncontrolled diabetes or uncontrolled blood pressure or uncontrolled high cholesterol.
There is no relation to meals.
Lung disease would present with a cough which can be dry or wet or shortness of breath or pain on breathing (though this can be due to other causes as well). Chest pain may be present in some cases.
There may be a history of smoking or previous lung conditions.
GERD is related to meals or types of foods used. There may be an acid taste in the mouth or excess salivation.
There may be bloating or nausea or vomiting. In some cases of reflux, there may be an associated dry cough.
Worse early morning or late at night.
