Question: Hi

I am 35 year old male weighing 102 kgs. I am hypertensive and am taking amplopress 10 mg. 10 days back i was also diagonsed with dyslipdimea with a bad cholestrol profile (reports attached) . I have family history of CAD and my father died aged 55 yrs. My Mother is also a caridac patient.

I am very anxious by nature and due to my Dad's death am very scared of well....dying. i was pretty active till physically 7-8th months back. It was in Dec 2014 i went litarrgic and gained weight.

10 days back when i saw my lipids report .My anxiety grew very high . I had an episode of sweating, pain in left arm and sinking feeling .Doctor did ECG and said it was fine. Although going by my history he ordered Echo caridograpgy and TMT test. Both the tests were normal

Doctor said i have no cardiac problem and need to control my lipids and change my lifestyle.

I will like to mention i am now sincerly following medication prescribed by doctor and working on getting back into my shape.

But at a times i get very anxious like i am going to have cardiac arrest.

I will like to ask How safe is if my echo, TMT are normal.?
What are chances of cardiac arrest ?

Doees Anxiety causes Heart disesase ?

I understand your concern and I would like to explain that according to current international guidelines on hypertension and cardiovascular risk prevention, as you suffer from hypertension and two other known cardiovascular risk factors (heredity for CAD, dyslipidemia), you result at a moderate risk for developing future cardiovascular events in the next 10 years.

This alarming situation imposes an immediate need for properly treating your modifiable risk factors ( hypertension, dyslipidemia); modulation of your lifestyle (avoid closed smoking contacts, engage on more physical activities, reduce weight, etc), and of course diet modifications to a healthy one ( I strongly recommend you to follow the mediterranean diet).

I would also recommend to control your anxiety as it doesn't result that you are suffering from an overt cardiac diseases.
Your cardiac ultrasound and the stress test have ruled out this possibility with a great safety.

The chances for you to suffer from a cardiac arrest are the same as every other healthy normal person.

So do not panic and relax!

But you have to exert vigorous efforts to avoid your coronary risk factors, maintaining in that way a very low probability for dangerous cardiac events.

Grateful for your elaborate answer . I will like to mention that i was a smoker for 2 years but i had quite smoking now from 8 years.i am how moderate alcoholic (have cut on alchol from last 1 month completely)

I am also suffering from hiatal hernia .Hope that is also not a problem ?

I would recommend you to avoid smoking and heavy or moderate alcohol intake in the future as they are lifestyle risk factors for coronary artery disease.

The past is not very important at this point, as you refer to have ceased to both smoking and alcohol. The most important thing is to avoid them in the future. The alcohol intake is also harmful to your hiatal hernia and reflux.

Regarding the hiatal hernia it is has been related to coronary artery disease as an inducer of angina in patients with gastroesofageal reflux and coexistent coronary artery disease. But not as a cause. I would recommend you to use regularly omeprazol for the reflux.
