What Are The Chances Of HIV Infection After Protected Intercourse?

Question: Hi doctor,

I had sex( only 1 episode ) with a commercial sex worker about 12 days back.
I used two condoms( one over the another).
There was no rupture or breakage of condom.

But i am still fearful of acquiring HIV.

This the the only time i had any sexual intercourse with anyone.

How should i proceed?
What should i do?
Is there any chance of hiv transmission.

Kindly answer my queries . Thank you.

I have a scheduled an appointment for western blot testing 12 weeks after the episode.

Should i start any antibiotic prophylactically to prevent any STD?

Thanks for writing in to us.

I have read through your query in detail.
Please find my observations below.

1. It is clearly mentioned that you had completely protected sexual intercourse and there was no break of condom. Therefore transmission of HIV is completely ruled out.

2. If she did not have any blisters in the skin of genital region then herpes is almost totally ruled out too.

3. Sexually transmitted diseases have a window period and if it is tested within the window period then chances are that the result will be false negative. Oral sex might transmit infections like syphilis and herpes but the chances are more if your partner had mouth rashes.

The window period for syphilis is 4 weeks after which tests will be accurate and for herpes the test is 70 percent accurate after 3 months. For chlamydia an gonorrhea the test is accurate after 2 weeks. You can get tested for syphilis after a month for accurate results.

4. I understand your anxiety is high at the moment, please relax and know that you had protected sex with your partner and therefore there is absolutely no chance of HIV infection. STD risk is low if you have had protected sex.

5. Taking prophylactic antibiotics is not required in your case as there is no symptoms that you are having. You can get STD tests done at 4 weeks and then take treatment if required.

6. For peace of mind you can get tested for HIV and results will be negative if you had protected sex. You can get HIV test by western blot at 12 weeks.

There are other tests also like Rapid 4th generation done after one month or 3rd generation ELISA test done at 3 and 6 months. All tests are highly accurate.

Dear Doc,

What are he tests that i should get done at 4 weeks?

I have done a fair amount of research.

I have already taken
T. Cefpodoxime 400mg stat for gonorrhoea
T. Azithromycin 1g stat and planning to take once weekly for another 2 weeks .

Have planned to take a shot of Benz.Penicillin 2.4 million units im tomorrow.

What i am worried about is candidiasis and trichomoniasis?
What is the prophylaxis that i should take for these two?

Dear doc, you may think i am a little paranoid.
But i really dont want this single mistake to screw up my life.

I am ready to do anything , to minimize the consequences as i am young and a family depends on me.

And one more query... Western blot done after 6 weeks for HIV is confirmatory right?

Thanks for writing in back with an update.

1. Please do not worry so much. I regularly have many patients like you and people get worried for no particular even after having protected sex.

2. For trichomoniasis the recommended medicine is metronidazole 400 mg 3 times a day for 5 days and must be taken only with a prescription from your doctor. Please do not take alcohol when taking medicines as it can cause harmful reaction. Candidiasis requires no preventive treatment.

3. From your history, there is no cause of worry as you do not have any symptoms and sex was protected.

4. However you might choose to get the following tests done at 4 weeks
(i) TREPONEMA PALLIDUM ANTIBODY (TPAB)
(ii) TREPONEMA PALLIDUM HAEMAGGLUTINATION
(iii) ANTI CHLAMYDIA ANTIBODIES IgG IgM
(iv) HERPES SIMPLEX VIRUS II (HSV) IgG IgM
(v) HEPATITIS B SURFACE ANTIGEN (HBSAG)

5. For HIV, it is recommended that you do
(i) HIV ELISA for HIV 1 and 2 antibodies at 3 months and western blot at 6 months for confirmation.

6. Rapid 4th generation test is confirmatory for HIV 1 and 2 antibodies and p24 antigen at 6 weeks. This test is not available every where.
