Based on the history you have shared, MS characteristically present with waxing and waning neurological symptoms, like history of neurological deficits like weakness, visual loss or any other impairment which may resolve and present again or a different set of symptoms over the years alternating or progressively increasing in each alternative episode etc.

Also that usually on these many MRIs some features like periventricular plaques or plaques in spinal cord or around basal ganglia etc may have been detected but the absence may not be enough to rule it out.

Also work up of any neurological symptoms to rule out other possibilities is a must. Occasionally oligocolonal bands or protiens rise in CSF examination may also be needed. Wishing you a healthy life ahead. Regards

My symptoms have been low back pain pain in legs fatigue the last couple of day having the urge to urinate are these symptoms of MS so mri are not good enough

Your leg weakness doesn't seem to be central causes, but lower legs motor and sensory system examination, may be helpful to differentiate between lower and upper motor neuron lesions. This will help to rule out MS. MRI spine to look for any signs of nerve compression should be done if previous scans were unremarkable. Levels for B12 and folic acid and D3 levels may be needed. For urine symptoms, urinalysis to rule out local infection may be helpful. Pre and post void urine volume on ultrasound needs to be measured to look for any signs of true retention. These will help to formulate a composite diagnosis. Wishing you a speedy recovery. Regards

The symptoms can be due to dosc herniation

Hi! Thanks for the details. Since EMG was normal, which tests the muscle fibres response and I hope that NCS or nerve conduction studies would also be normal, which means that no gross motor dysfunction or any signs of peripheral lesion to nerve has been ruled out but still disc compression can cause such subjective weaknesses.

Apart from that, we had already discussed other possibilities of such weaknesses and the levels of B12 and folic acid alongwith calcium and vitamin D3 levels may be reviewed. Thyroid profile and sugar levels may also be needed.

The work up is multidimensional and careful examination and follow up can narrow down your physician to diagnose. But the likelihood of MS is not evident through any of the workup and may need those tests which we had discussed earlier.

Since urinalysis was normal, other causes of over active bladder needs to be worked up. A urologist review and micturation studies or pre and post void urine volume estimation may also be needed. Wishing you a speedy recovery. Regards

As in these many years with such neurological deficit, either symptoms or scan findings would have worsened or would have exhibited some other neurological sign by now. Which is how MS is mostly diagnosed as in MacDonald criteria.
