What Are Some Of The Factors That May Cause Fluctuations In INR Values?

Question: I have been taking 5mg (4and1mg) daily for a few months and am now
experiencing dizziness and unstady symptons. Also slight headaches, my inr tests fluctuate from 2.8 to 2.3. can you throw some light on this please.

Hello,

please inform about the name and dosage of the drugs you use. For example Lipitor 10mg once per day.

If you're asking about INR fluctuations, this is normal. The INR and anticoagulant effectiveness are influenced by so many factors, that you can never be sure about the future INR. That's why monitoring is advised.

What you can do is to avoid taking any medications besides your regular ones and try to avoid daily changes to your diet, particularly regarding vegetable intake. For example, if you eat a salad every day but - for any reason - you stop eating it for a few days, your INR could rise and vice versa. Getting sick and taking antibiotics may also cause your INR to fluctuate. The same applies for liver disease. Liver dysfunction usually causes the INR to rise and makes it more difficult for patients to achieve the INR targets.

I was hospitalized with lung infection/pnumonia last april and discharged
about 2 weeks later.
the medications I am on are:
90 Diltiazem HCI ca 240mg cd (apo apo), . Plus
200 Warfarin sod Ta 1mg and 150 warfarin 3mg,all one per day.
90 simvastatin 20mg. I take all these at night,
in the morning I take
90metoprolol succ ta 47.5mg cr ,met aft and
90candesartan cilexetill ta 32mg can myl
both one of each.
my symptons are unsteadyness, no appetite, thirst, headaches for wich I take paracetimol., trembling when cold, slight nausea.
pulse rate is 70, sometimes a little higher

thanks for the additional info.

thirst is a symptom of low blood volume, which is usually caused in dehydration, blood loss or other reasons of a decreased intravascular volume (diuretic use etc). You don't seem to have such a cause. Your drugs are not supposed to cause such symptoms unless your blood pressure is very low. Have you been drinking enough water or other fluids? A useful

Dizziness and unsteadiness can be caused by many drugs (including some of yours) particularly when the blood pressure is low or low-normal.

Common causes of 'general' symptoms like the ones you've mentioned include anemia, electrolyte disorders, thyroid problems, etc

Obviously I can't get into more details without examining a patient but I can tell you some important points to check or watch.

- you've got to check your temperature to make sure no infection is active.
- you've got to check your blood pressure and pulse rate to make sure they are not very low.
- drink enough fluids in order to make your urine look more like water rather than dark yellow tea.
- you can check your hematocrit and electrolytes to detect anemia or electrolyte disorders which may cause similar symptoms.
- check your TSH to exclude thyroid disease.

If none of the above applies to your case then you should contact your primary care physician for a more detailed approach.
