Please go through detailed answer dear.

Detailed Answer:
Hello and thank you for choosing "Ask a Doctor" service for your query,
Have seen your details.
By piles i am taking that both Digital rectal examination(External hemorrhoid) as well as anoscopy (Internal hemorrhoid)was done by your doctor.
As you are having a lower back pain from few month i would suggest you to go for few tests to rule out pelvic inflammation(common in females). However usually rectal bleeding is not there.
Tests suggested-
Complete blood count
CRP and ESR.
Again as bleed is there a second check examination by anoscope could be repeated to rule out any small internal hemorrhoid(Usually present with bleeding) and can be missed sometimes in grade 1.
Follow up with results.
Thanks!
