Question: To be answered by Dr, Spaho. Hello, doctor. Since you are very familiar with atypical odontalgia and have suggested Botulinum Neurotoxin injection (still searching for a doctor who does this) - the gingiva between the two teeth has been cut several times and now, there is an actual hole which of course, exacerbates that gnawing non-stop ache. My question is - am I less of a candidate for success with the injection because the area has been so traumatized or does it make no difference to the outcome? Thank u for answering that and just a quick follow up - this pulling, gnawing aching - is this coming from a cranial nerve or trigeminal? Again, appreciate your invaluable insight.

Hello again and again thanks for the question.

Trigeminal nerve is a cranial nerve.

In cases of atypical odontalgia the peripheral branches of the trigeminal nerve are irritated, so, resulting in pain.

Botulinum toxin injection controls the irritation of these nerves resulting in less pain.

So, in your case, the pain that may originate from gingiva should be improved too.
