Question: Thumb infected for over 15 years due to excessive skin picking and peeling...no cuticle left always swelling nail plate risen bumpy and feels like something underneath trying to bulge out. Filed it down and it peeled top layer skin bubbling up from opening coming out like red bubble mass. From top center not the sides. Painful throbbing. Thumb has always hurt worse in the cold weather. Same on left thumb but not as severe and never punctured.

Most likely it seems that there is soft tissue infection around the nail plate i.e paronychia. I suggest you to take an oral antibiotic e.g either Amoxicilin or cephalexin for a week. In addition I suggest you to take an oral anti-inflammatory e.g either ibuprofen or diclofenac for a few days. These are prescription medications.

Pyogenic granuloma

Thank you. I have reviewed the image.
It is actually a periungual pyogenic granuloma which is due to localized capillary proliferation presenting as a vascular growth.Most likely it is due to local trauma (of skin picking and peeling). I would advice you to get it cauterised. Take care.
