Question: This is my story:

Enlarged prostate for a long time, voided 1.8 liters in the emergency room this past December. Went on a Foley for two months not realizing this would make a lazy bladder. Started self-cathing 1 week ago, but have TURP surgery in 4 days (80g prostate).

So I’ve made two mistakes with my bladder and I realize a third mistake could kill me. Most recently, I only started self-catheterizing a week ago leaving my bladder to get lazy for two months with a Foley.

I am afraid that if I have the surgery, I will have an inbound catheter for a month or two after that, further killing my bladder so that it never recovers and I have to self-catheterize for the rest of my life. Or worse, if the surgery interferes with self-catheterizing I will have an inbound foley until I die from infections.

So should I keep self catheterizing until my bladder recovers, then have the surgery, and hopefully need the foley for only a short period, or have the surgery anyway?

Thanks for query .
Based on the facts that you have posted you seem to have Benign Enlargement of Prostate since a long time .This causes obstruction to bladder outflow of urine .As a natural phenomenon bladder tries to pump out the urine agains the resistance of enlarged Prostate making it to be hypertrophied and gradually over a period of months or year gets tired itself to become non complaient and the power of force of contraction of bladder gets reduced causing chronic retention of urine .This explains the fact that about 1.8 liter of urine was drained out in Dec 2021.

I would explain importance of long term Foley catheterization .When one gets tired we take rest to regain our energy .The situation is similar in such cases of non complaaient bladder .Though removing the obstruction at the bladder outlet by doing surgery of resection of Enlarged Prostate is the primary objective of the treatment the power of the bladder contraction of bladder being redduced one is unable to pump out the entire urine from the bladder even after successful surgery.So it very important to gain the power of contraction of bladder which is achieved by putting the indwelling Foley catheter to drain out the urine This self retaining catheter is left in for couple of weeks or even a month so that bladder gets complete rest This helps to regain adequate power in bladder to pump out the urine .Hence surgery is deffered till your bladder recovers completely .This is confirmed by doing Urodynamic Studies of Bladder .

Putting a cathter and keeping it for long period is helful and mandatory to regain bladder power and it does not cause any harm at all..I would suggest to keep the Foley catheter for couple of months .The plan of action should be as follow
1) Keep the catheter for a month
2) Remove the catheter and do Urodynamic study to assess power of Bladder .
3) If the study shows power to be still inadequate put the catheter again for a month and repeat Urodynamic ecvaluation again after one month . Normally it takes 2 mnonths for Bladder to regain power .
4) Once the study shows adequate improvemnt in bladder power then only it is advisable to do TURP and .the rsults of the surgery are outstanding and no catheterization is required for more than 3 days after surgery .
5) This protocol is a standard modality followed world wide .
6) Doing surgery in hassel and in hurry makes the situation more complecated and one may not be able to pass urine .
