In modern toxicological testing today, the positivity of the test shall depend on the source. Most people get Arsenic from contaminated water sources. If you are in contact with the contaminating source, it is possible that the test remains positive.

But if you were once exposed or accidentally got some quantity, and stopped, it is very impossible for it top test positive after 20 years. You will not be able to find it at this point, its too long and cannot stay up to measurable levels for this length of time.
