Question: Hello Doc,
I am 28 and I have some trouble at bed. I ejaculate very soon. I have high sexual desires and I used to masturbate 2 times everyday. Not sure if that's the reason.

I need some help to increase my sexual strength.

You are likely suffering from premature ejaculation.

First of all clarification about masturbation.

Masturbation is natural and normal thing. It is done by 95% of the males in the world, mostly before marriage as well as when wife is away, in menses or pregnant after marriage. It is not at all harmful. Frequency of masturbation done by an individual varies from once a day to once a week. It depends upon individual libido, wish and timing. It neither causes any weakness nor any problem in sexual activity.

However masturbation and sex are different techniques for reaching to same goal so initially person who used to enjoy masturbation may face difficulty in sex.But it is natural phenomenon not a disorder.
Initially many young men when begin sex life face such problem. This is likely due to initial experience of sex, situation in which sex is done, over excitement with partner and sensitivity glans penis.

Let me know some information.

- Are you married or not? If not married have you experienced sex with girlfriend or with someone else? Tell me your experience of sex with brief details.

- Were you apprehensive about situation and in hurry while doing sex?

Premature Ejaculation (PME) defined when male partner ejaculates early before penetration, just after penetration or the female partner reaches to orgasm. It should last at least for 1 minute after penetration.

Treatment for PME is there:

Medicines:
Clomipramine and paroxetine are choice of medicines which are used as antidepressants (For depression dose is higher).
I prefer clomipramine for PME. No significant side effects with these medicines. Initially say for 20-30 days daily after dinner and later on just on the day when you do the sex.
Paroxetine is more potent but it may cause side effects like headache, gastric discomfort, giddiness and sedation. In patients with blood pressure it should be used in consultation with physician/cardiologist.

One molecule called depoxetine has been used specifically for PME and should be taken before 2 hours of sex when required.
These medicines are prescription medicines and you need to consult doctor/psychiatrist/sexologist for prescription.

Behavioral approaches are as follows for PME:

1. Stop & Start and Squeeze techniques can be applied which you can learn from internet and ask me if any query in that.(In brief, method suggests that you should try to divert your mind for a moment when you feel ejaculation arousal or penis should be tightly squeezed with hand inwards by your partner preferably just before arousal).

2. Sex counselling of your sexual behavior and methods. Like if you are down and female partner on top, gravity is going to help you for lengthening the time.

3. You should do masturbation before sex (at least 2 hours before ). It will make seminal vesicles somewhat empty and at time of sex semen will take more time to come out.

4. Use of condoms. Sometime use of condom while sex helps in delaying ejaculation by reducing sensitivity of glans penis.

Doc, Thanks for the detailed explanation. I am married and regarding the experience, we do the foreplay for sometime and when I try to penetrate, I couldn't stand for more than a minute if I do slowly but if I do it fast.. it's even less. But my partner likes it to be fast. sometimes when I feel I am going to ejaculate .. I'll take out and do some foreplay and penetrate again.. which buys me some more time. And with the condom on, I am able to stand more than the time I stand without condom. I feel very embarrassing that I couldn't satisfy my partner. So what I understood from your message is reducing masturbation won't help me? Do you suggest I need to consult a doctor? if yes, what kind of doctor? urologist or sexologist? I mean the speciality.

You did not mention duration of marriage life as I did not ask specifically.
However if you are newly married (less than 3 months) you will be normal soon with experience. You are already following some of behavioral approaches I have mentioned and you also succeeded.

I suggest doing masturbation before 2 hours of sex, it will help. Yes reducing masturbation will not help.

But if you are married more than 3 months and yet your wife want more time you can take help of medicine like clomipramine as I mentioned.

The best specialist to consult for your case is psychiatrist. Psychiatrist is by degree a sexologist also and used to deal with such problems. He would hear you patiently and guide you in right direction.
