Question: My two yr old has recently been waking up at night or in her sleep screaming and crossing her legs so tight. She doesnt seem to eat certain things like she used to nor perfur her milk. No fever or vomiting. But acid reflex these past two days seems worse. I have taken her to doctor after doctor bc she has a history of holding in her bowels. But she is not constipated. What should i watch for that could be serious?.. Should i be concerned about this? Or is it just a tummy ache?... I want a scan done but everydoc just says it is behavioral

Brief Answer:
Worms

Detailed Answer:
Hi,

Thank you for consulting in Health Care magic.

I understand your concerns regarding your kid's pain. It could be worm infestation. Usually this happens in this condition. We can confidently rule out non-organic pain as the age is not fitting and at the same time, this is occurring during sleep.

Regarding your questions -

1. I can see the kid is suffering. I will be more worried if she has vomiting which is green in color, blood in stools or weight loss or rash over the body. These are the danger signs which you need to be worried about.

2. You told she has the habit of holding her bowels, but she is not constipated. This is suggestive of "Voiding Dysfunction". I suggest you get an Ultrasound scan done with this provisional diagnosis in mind. But this requires a pre and post void scan.

3. Kindly get back to me with answers to these questions -
a) Did she have urinary infections in the past
b) Is there a family history of constipation
c) Is there a possibility that during play she might have introduced a toy part into her

4. This doesn't look like a simple tummy ache. Please do not ignore.

Hope my answer was helpful for you. I am happy to help any time. Further clarifications and consultations on Health care magic are welcome. If you do not have any clarifications, you can close the discussion and rate the answer. Wish your kid good health.

But you mentioned a rash... What type of rash would i look for abd what type of worms are u thinking?

Brief Answer:
Worm infestation

Detailed Answer:
Hi.... Thank you for the trust in me.

Rash - Look for red, papular rash which can be felt by hand on touching.
It need not be itchy.
It might come predominantly in the lower limbs.

Worms - These can be pin worms or seat worms as they are commonly called.
These worms have a life cycle in such a way that they come out to lay
eggs in and around anus and this could manifest like this.

voiding dysfunction - It may present like this and this is confirmed only by
ultrasound abdomen. But the symptoms will also include
inability yo hold back the urine once the sensation starts.

If can get a scan done and if ti is normal, then you can stay assured that nothing serious is going on in the kid. You can give a try of de-worming with Albendazole once dose of 200mg today and another dose of 200mg 10days later.

Ok im researching worms and she does display some symptoms, however before i first ask the question last night, I gave her those children's anti-acid meltaway's right before she went to sleep (she takes her previcid in am. She winned a few times in the night but nothing to wear i had to go in her room bc she went strait back to sleep. With this said would u still lean towards tge worm dx.? (I also need to mention my two yr okd has a 18-q deletion and was on growth hormones till about 3 days ago and will be getting back on them asap (onsurance issues) but she has had no negative effects with them has bren on since 6 months

Brief Answer:
Please treat worm infestation

Detailed Answer:
Hi... Even then i adivse you to treat worm infestation and as you have mentioned now about 18q deletion and growth hormone therapy, I advise you consult your endocriniologist too in this.

But i assure you that there is no drug interaction with Albendazole and growth hormone.
