Suggest Treatment For White Coat Syndrome

Posted on Mon, 17 Aug 2015
Question: Hi I am a 54 year old male with hypertension. My doctor has put me on 3 different drugs. I do get what's called white coat syndrome prior to blood pressure check causing it to elevate beyond my normal reading which is 130/80 to 140/90. I have had stress test cardio / heart ultrasound etc. all normal however I do have slightly larger left ventricle. My daily medication dosage is Noten/Atenolol 50 mg twice a day. Atacand 32/12.5 Once a day and have now also been prescribed Prazosin 5 mg twice a day. My question is that I am concerned with the amount of different medication. Would it be safe for me to slowly stop taking the Atacand 32/12.5 and just stay with Atenolol and Prazosin. A second professional opinion would be greatly appreciated. Regards, XXXX.

Brief Answer:
I need details.

Detailed Answer:
Welcome To HCM. I am Dr Anshul Varshney. I understand your concern.
To help you better, I need to know a few more things:
1. What is your BP reading whenever checked on medications and off medications (irrespective of white coat Hypertension)?

2. What were your BP reading for the first time when you were started on anti Hypertensive drugs?

3. Initially you were on which medicines? Why your doctor need to increase the dose and number of medicines?

Revert with the asked details for a better help.

Hi Dr. XXXXXXX

Thank you for your reply. I have been on blood pressure medication for about 10 years. Initially starting with Avapro which did not suit me. Later I was prescribed Noten / Atenolol 50 mg eventually twice a day and Coversyl 10 mg once a day.
My initial reading prior to starting medication 10 years ago was around 150/90.
3 years ago I went through severe financial stress due to business collapse and was driven to hospital with blood pressure reading 200/120. Since then with different increased medication my blood pressure is still anywhere usually between
140/90 to 170/103 when very anxious or stressed. my concern is why can't my doctor find the right drug for me instead on constantly increasing. All blood tests are good apart from slight cholesterol. kidney function good.
Regards, XXXX.

Brief Answer:
Carry on with that

Detailed Answer:
Hello, If your BP readings were so high as mentioned in follow up, you would require to take all the medicines as advised by your doctor.
Along with that adopt life style changes and we will reassess you for further reduction in doses if possible.
I would advice you following for life style modifications:
1. Reduce your weight. Start exercising regularly particularly brisk walk.
2. Avoid Fatty food in diet.
3. To consume potato, rice, oil, ghee, sweets, carbonated beverages in moderated amount only.
4. Avoid alcohol and smoking in case you do.
5. Take low salt diet and avoid pickle, salt in diet.
Write back to me after 3 weeks of adopting these life style changes with the fresh BP readings.
I hope it helps.
Still, if you have any further query, you may please ask me.
Stay Healthy.

Thank you. I was hoping that perhaps a different drug could possibly replace my current medications. However I guess lifestyle changes is a good starting point. Thankyou, XXXX.
