Question: I am having cough and have to remove wet cough periodically I feel irritation in neck and cough starts

I can understand your concern.
By your history and description, possibility of either worsening of asthma is more.
Another possibility is RA associated Pulmonary parenchymal involvement like fibrosis.
So better to first get done PFT first.
Also get done HRCT thorax for the diagnosis of fibrosis.

2. Which drugs you are taking for asthma and RA?

Start n acetyl cysteine.

Thanks for your follow up question on HCM.
I can understand your concern.
So you ate taking optimal inhaled treatment.
Wet cough is mostly due to either infection or inflammation.
So short course of broad spectrum antibiotic and n acetyl cysteine (NAC) will be helpful in your case.
NAC is having mucolytic, expectorant and antioxidant properties.
Please let me know
1. Since how many days you are having these symptoms?
2. Have you ever undergone PFT and HRCT thorax?
3. Do you have fever?

Three times HRCT was done
after First HRCT sample pleural effusion was extracted to know the infection
After analysis bacterial infection was found
Then to reduce pleural effusion dose of wyson steroid was given
Then again HRCT was done along with sonography
It was found to be minimal so regular treatment was started

Start NAC. It will definitely help you.
Also start deep breathing exercises and chest physiotherapy.
Both are good for lung functions.
Also vaccinate yourself with pneumococcal and Influenza vaccines to prevent recurrent respiratory tract infection.
