Question: EBV and CMV IGG positive...hi i am XXXXXXX from XXXXXXX .i had a risky sexual encounter in april 2014 in XXXXXXX red light area gb road.i had all kinds of symptoms - weakness,fatigue,pimples on the face,weight loss,pharyngitis,itching in the rectum area, low blood sugar levels in the morning,sudden hotness in the body,am getting cold and sore throat after every 2 months..weakness is so much that after doing a little bit of hard work i feel very much tired..after testing hiv for 9 months and all tests were negative..including 4 pcr tests and 20 antibody tests..i went ahead with cmv and ebv virus antibody test...while cmv showed IGG reactive with value of 95.5 and EBV came reactive with IGG value 119.so i wanted to know..1.is it cmv and ebv which are causing all my symptoms..2. An april 2014 encounter..infections got during that period will comw under IGG?..3.is there any treatment for these viruses in XXXXXXX and any good doctor in delhi?..4.how to deal with this weakness,low energy on daily basis..5.till how long will these symptoms last.6. Do i still need to test hiv or its not required......waiting for answer on these questions.

I have gone through your query and I have understood it.

IgG positivity for CMV and EBV just means that these are past infections/ latent infections. It does'nt tell anything about when you got these infection.

Probably you already had them before this particular sexual encounter.

Antibodies to these viruses may be present in otherwise healthy individuals as these virus are widely distributed and detection of antibodies don't mean that you need to be treated.

I don't think that your symptoms are due to these viral infections, more so because you are Hiv negative.

These viruses may cause a transient 'flu-like illness' in those who get exposed to infection, which is usually the case with most viral infections.
However, these viruses don't usually cause symptoms on a regular basis (i.e every 2 months) in immunocompetent individuals.

There is any need to treat for these viral infections in an immunocompetent
individual..

These viruses can re-activate in immunocompromised individuals e.g HIV positive individuals and cause systemic symptoms.
Only then there would be a necessity to treat these infections with antivirals, not otherwise.

Since HIV has been adequately ruled out by blood tests (pcr and antibody based tests), therefore there is no need for any further Hiv testing and it is enough to establish that you are Hiv negative.

Regarding the weakness, I suggest you to consult an internal medicine specialist to detect an underlying cause(s) other than these.
Your doctor may order a series of tests including blood, urine and X-rays in an attempt to find out the cause for these symptoms.

Your symptoms are certainly not due to either CMV nor EBV or HIV

Thanks for the answer..but i hav got mostly everythin checked...my cbc is showing increase in lymphocites and TLC from april 2014 onwards after my encounter...my lymphocite count which was 30 in may 2014 is 47 in november 2014..so i knw ther has been a viral attack..plus the cd4/cd8 ratio which shud be greater then 1 for a normal human bieng is presently at 0.70..with cd4 at 1100 and cd8 increased to 1800...the normal range for which is around 1000...plus symptoms hav bugged me alot..as earlier never these kind of symptoms were faced by me...

HIV is out of question.
Raised blood Lymphocytes can be due to numerous causes like infections (viral, bacterial) and malignancies like lymphomas.
Absolute count of lymphocytes and WBC's is more reliable rather than percentage.

-Please let me know what was the total WBC count/ TLC ?
-I would like to know whether are there any enlarged/ palpable lymph nodes?
-Were you subjected to certain other tests like X -rays, Mantoux test/ TB skin test?

Hi...my TLC count in november was 10.1and lymphocytes were 46.70%..ther r no enlarged lymphnodes..doctor has done chest x ray..but nothin was found...fatigue has been lingering everyday..now also i camw back from washroom in the morning..i am feelin so weak...also early morning i feel so low in blood sugar..that after eating somethin sweet i get fine..so many symptoms doctor.

Hi.

Your total lymphocyte count is around 4700 (46.7 X 10.1) whereas the normal lymphocyte count is around 4000, that means there is absolute lymphocytosis i.e raised lymphocyte count.

Viral infections are a cause of acute lymphocytosis rather than persisting lymphocytosis.
In adults a persistent lymphocytosis is commonly due to tuberculosis.

Tuberculosis which is a quite prevalent in XXXXXXX is also a common cause of weight loss, tiredness and low grade symptoms and can also present with lymphocytosis.

I hope you have been investigated appropriately for extra-pulmonary Tuberculosis as well.
Though your chest X ray is normal and therefore unlikely that you have pulmonary tuberculosis but tuberculosis can affect other tissues like lymph nodes, abdomen, bones etc and therefore a mantoux test should be done along with an Interferon-gamma release assay (IGRA) (which is a blood test) for TB infection.
The results are usually available in 24 hours.

Apart from that a physical examination for any enlarged lymph nodes, if present should be followed by a fine needle aspiration cytology (FNAC) and even lymph node biopsy.

I guess you should seek another appointment with an Internal medicine specialist/ infectious disease specialist.
